Spring 2025 Clinic Upper DivisionThis course assists students to develop and broaden clinical judgment and skills. Content includes the study of chronic illness and recurring conditions in adolescents and adults and the impact of those conditions on family members. Focus is on the differential diagnosis, clinical decision making and management, as well as patient and family education within the context of primary care. The adult nurse practitioner role is analyzed in the context of caring for persons with chronic conditions. Professional issues, collaboration, scope of practice and advocacy are emphasized.
Learning Outcomes1Apply epidemiology, developmental, physiology, informatics, technology, leadership, systems, policy, quality improvement, family, community, and health promotion theories and research in the care of adolescents, adults, and older adults with chronic conditions.
2Use appropriate diagnostic modalities in assessing the chronic health care needs of adolescents, adults, and older adults.
3Synthesize clinical data to formulate, implement and evaluate care for chronically ill clients and their families.
4Implement primary, secondary, and tertiary prevention incorporating quality of life and financial considerations when providing health care to adolescents, adults and older adults with chronic health care problems.
5Use appropriate ethical and evidence-based practice standards in the management of chronic physical and mental health problems in adolescents, adults, and older adults.
6Demonstrate comprehensive management of the primary care needs of adolescents, adults, and older adults from diverse backgrounds with chronic, recurring conditions.
7Utilize community resources in the primary care of individuals and families with chronic conditions.
8Integrate the teaching-coaching role into the management of chronic illnesses of adolescents, adults and older adults.
9Implement interdisciplinary collaboration and APN scope of practice while providing safe treatment to adolescents, adults, and older adults.
10Address the uniqueness of diverse populations in the provision of safe health care to adolescents, adults, and older adults with chronic conditions.
