Spring 2025 Lecture Distance Learning Upper DivisionExamines strategies various industries use to assess the risk of organizational accidents and to develop safety management programs to prevent, capture, and recover from conditions that lead to disastrous outcomes. Strategies such as High Reliability Organizations, Operational Risk Management, Behavioral Based Safety, Tripod Delta, and Safety Cultures are explored as successful methods for improving organizational safety in high-risk environments and endeavors. Permission of instructor required.
Learning Outcomes1Explain Reason's model of Organizational Accidents (OA) and its application in understanding accident causation and prevention.
2Analyze everyday operational events through the lens of the OA model to identify latent risks and contributory factors.
3Evaluate best practices from safety frameworks such as High Reliability Organizations, Operational Risk Management, and Safety Cultures to mitigate the risk of organizational accidents.
4Develop a detailed, evidence-based analysis of selected aviation accidents or incidents, incorporating the principles of the OA model.
5Propose strategies to enhance organizational safety by integrating behavioral-based safety and proactive risk management techniques into safety management systems.
