Fall 2025 Lecture Distance Learning Departmental CreditUpper DivisionExplores the definition and nature of human error, error chains, and causal factors in error generation. Error taxonomies are reviewed in order to provide a classification scheme for grouping errors and assessing error criticality. Methods for assessing risk and predicting error generation potentials are learned, as well as strategies for controlling or eliminating errors. Case studies are utilized throughout the course to illustrate course concepts. Permission of instructor required.
Learning Outcomes1Classify and evaluate types of human error using established taxonomies and assess their criticality in complex systems.
2Analyze the antecedents and causal factors of human error, including those related to equipment, operators, organizational practices, regulatory frameworks, and cultural influences.
3Apply error investigation methods, including data collection and analysis, to identify patterns and inform risk reduction strategies.
4Examine case studies critically to propose evidence-based strategies for mitigating error incidents and improving safety outcomes.
5Develop and present a scholarly research project on a contemporary human factors issue, demonstrating effective written and oral communication skills for professional contexts.
